Samantha Maiden, Online political editor | October 09, 2008 SIGNALLER Sean McCarthy died in Afghanistan from massive wounds and "no equipment or personnel could have saved him", a defence force inquiry has found. The Australian Defence Force's report into McCarthy's death finds he died from "massive wounds sustained when an improvised explosive device detonated under the Long Range Patrol Vehicle he was travelling in". No further investigation will be held in accordance with advice from The Chief of Defence Force, Air Chief Marshal Angus Houston.
